Follicle-Stimulating Hormone (FSH) is a pituitary glycoprotein gonadotrophin (alpha-beta heterodimer) secreted by the anterior pituitary gonadotroph cells in response to hypothalamic GnRH (gonadotrophin-releasing hormone) stimulation, regulated by negative feedback from gonadal sex steroids and inhibins. In females, FSH drives folliculogenesis — stimulating granulosa cell proliferation, oestradiol production (via aromatase), and expression of LH receptors on the dominant follicle — and is the primary driver of follicular growth during the follicular phase of the menstrual cycle. FSH surge in coordination with LH surge triggers final oocyte maturation and ovulation. FSH is a critical marker of ovarian reserve; elevated basal (Day 2–3) FSH (>10–12 IU/L) indicates diminished ovarian reserve and predicts poor IVF response. In menopause, FSH rises markedly (>25–40 IU/L) due to loss of gonadal inhibin feedback. In males, FSH stimulates Sertoli cells to support spermatogenesis; elevated FSH in a male with azoospermia indicates primary testicular failure (non-obstructive azoospermia — Sertoli cell-only syndrome, germinal aplasia). FSH is essential in the workup of: amenorrhoea (elevated FSH → primary ovarian insufficiency; low/normal FSH → hypothalamic/pituitary cause); infertility (male and female); precocious puberty (elevated for age confirms central/gonadotrophin-dependent precocious puberty); delayed puberty; hypogonadism; and pituitary adenoma evaluation. FSH should be ordered alongside LH, oestradiol/testosterone, prolactin, and AMH for complete reproductive endocrine assessment.
