Test Description

17-Hydroxyprogesterone (17-OHP) is an intermediate steroid hormone in the biosynthesis of cortisol and androgens in the adrenal glands. It is produced in the adrenal cortex and gonads from progesterone. Measuring 17-OHP is the primary diagnostic test for congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency (CYP21A2 gene mutations), which is the most common cause of CAH (90–95% of cases) and the most common adrenal disorder in childhood. In 21-hydroxylase deficiency, the enzymatic block causes accumulation of 17-OHP (and other precursors) with deficient cortisol (and aldosterone in severe forms) and excess adrenal androgens. Classic CAH presents in neonates with salt-wasting crisis (electrolyte imbalance, shock) or virilization of female genitalia. Non-classic (late-onset) CAH is milder and presents in adolescence or adulthood with premature pubarche, acne, hirsutism, or menstrual irregularities. Normal morning 17-OHP: early follicular phase females and prepubertal children <200 ng/dL; males <220 ng/dL. Values >1,000 ng/dL are diagnostic of classic CAH; values 200–1,000 ng/dL require ACTH stimulation test. 17-OHP is part of newborn screening in many countries.
