Test Description

Aldosterone is the principal mineralocorticoid hormone secreted by the zona glomerulosa of the adrenal cortex, primarily regulated by the renin-angiotensin-aldosterone system (RAAS) and serum potassium. Renin is a protease enzyme secreted by juxtaglomerular cells of the kidney in response to reduced renal perfusion, hypovolemia, or sympathetic stimulation. The aldosterone-to-renin ratio (ARR) is the primary screening test for primary hyperaldosteronism (Conn syndrome), the most common secondary cause of hypertension, accounting for 5–15% of hypertensive patients. In primary hyperaldosteronism: aldosterone is elevated (>15 ng/dL), plasma renin activity (PRA) is suppressed (<0.5 ng/mL/hr), and ARR >30. Causes include aldosterone-producing adenoma (APA) and bilateral adrenal hyperplasia. Symptoms: difficult-to-control hypertension, hypokalemia (though up to 50% are normokalemic), metabolic alkalosis, and hypomagnesemia. Low aldosterone with high renin suggests secondary hyperaldosteronism (renal artery stenosis, heart failure) or adrenal insufficiency. Measurement requires specific sampling conditions.
