Test Description

Urine Microalbumin measurement detects low concentrations of albumin in urine (30–300 mg/day or 20–200 µg/min), below the threshold detectable by conventional urine dipstick protein tests. Microalbuminuria is defined as urinary albumin excretion of 30–300 mg/g creatinine on a spot urine sample, or 30–300 mg/24 hours on a timed collection. It is the earliest clinical marker of diabetic nephropathy, preceding overt proteinuria by years. In patients with type 1 or type 2 diabetes, persistent microalbuminuria predicts progression to diabetic kidney disease (DKD) and is an independent cardiovascular risk factor. Microalbuminuria also occurs in hypertensive nephropathy, metabolic syndrome, and in the context of acute illness. Annual screening for microalbuminuria is recommended in all diabetic patients from diagnosis (type 2) or 5 years after diagnosis (type 1). Early detection enables timely initiation of renoprotective therapy with ACE inhibitors or ARBs, which can slow progression. Results should be confirmed on at least two of three samples collected over 3–6 months, excluding transient elevations due to exercise, fever, or UTI.
