Urine ketone testing detects acetoacetate (and to a lesser extent, acetone) in urine, the two ketone bodies detectable by the standard nitroprusside reaction on urine dipstick. Note that beta-hydroxybutyrate — the predominant ketone body in early DKA — is NOT detected by standard dipstick tests, which may underestimate the degree of ketosis in early DKA. Ketones are produced when fat is the primary metabolic fuel in states of inadequate glucose availability or impaired glucose utilization: Diabetic ketoacidosis (DKA): life-threatening complication of type 1 diabetes (and occasionally type 2) characterized by hyperglycemia, ketoacidosis, and dehydration. Starvation ketosis: prolonged fasting or severe caloric restriction (also ketonuria in normal pregnancy due to increased fat metabolism). Alcoholic ketoacidosis: chronic alcohol abuse with recent cessation and poor nutritional intake. Ketogenic diet (intentional nutritional ketosis). Isopropyl alcohol poisoning: causes ketonemia without acidosis. Urine ketones are graded as negative, trace, small (+1), moderate (+2), large (+3), or very large. In DKA monitoring, point-of-care serum beta-hydroxybutyrate measurement is now preferred as it directly measures the predominant ketone. Ketonuria typically clears before hyperglycemia resolves in DKA treatment monitoring.
