The Urine Albumin-to-Creatinine Ratio (ACR) is the preferred method for quantifying urinary albumin excretion from a spot urine sample, correcting for the dilution effect of urine by expressing albumin relative to creatinine concentration. ACR eliminates the need for timed (24-hour) urine collections, making it more practical in clinical settings. ACR is expressed in mg/g or mg/mmol. Reference ranges: Normal <30 mg/g; Microalbuminuria (moderately increased) 30–300 mg/g; Macroalbuminuria (severely increased) >300 mg/g. ACR is the cornerstone of CKD staging alongside eGFR in the KDIGO classification and is used for diabetic nephropathy screening, hypertensive nephropathy monitoring, and risk stratification for cardiovascular events.
