Test Description
Typhoid IgG and IgM serology detects antibodies against Salmonella typhi and Salmonella paratyphi, the causative agents of enteric fever (typhoid and paratyphoid). IgM antibodies appear early (Days 3–7) and indicate acute/recent infection; their presence at high titres is strongly suggestive of active disease. IgG antibodies appear later (Days 7–14) and indicate either past infection or vaccination-related immunity; their presence alone does not confirm active disease. The combined IgM/IgG profile, typically measured by ELISA or immunochromatographic assays targeting Vi antigen, LPS O antigen, or flagellar H antigen, provides better sensitivity and specificity than the traditional Widal slide agglutination test. Typhoid fever presents with sustained high fever, headache, bradycardia relative to fever (Faget's sign), rose spots on the trunk, abdominal pain, splenomegaly, and a "step-ladder" temperature pattern. Complications include intestinal perforation (most feared), haemorrhage, hepatitis, myocarditis, and encephalopathy. Blood culture remains the gold standard for diagnosis and antibiotic sensitivity testing. Serology is used when blood culture is not feasible or has been negative in treated patients. IgG/IgM serology should be interpreted in the clinical context of fever duration, endemic exposure, and vaccination status.
