Total Testosterone measures the aggregate concentration of all testosterone fractions circulating in the blood: free testosterone (~2–3%, biologically active), albumin-bound testosterone (~54%, weakly bound and potentially bioavailable), and SHBG-bound testosterone (~44%, tightly bound and biologically inactive). Testosterone is the primary male sex hormone produced by Leydig cells of the testes (95%) and adrenal cortex (5%); in females, it is produced by the ovaries, adrenal cortex, and peripheral conversion. Normal ranges are sex- and age-specific and show significant diurnal variation (highest between 6–10 AM, lowest in the evening). Male reference range: 270–1070 ng/dL (9.4–37.1 nmol/L); Female reference range: 15–70 ng/dL (0.52–2.43 nmol/L).
