Total Testosterone measures the aggregate concentration of all testosterone fractions circulating in the blood: free testosterone (~2–3%, biologically active), albumin-bound testosterone (~54%, weakly bound and potentially bioavailable), and SHBG-bound testosterone (~44%, tightly bound and biologically inactive).
