Test Description

Scrub Typhus serology detects IgM and IgG antibodies against Orientia tsutsugamushi (previously Rickettsia tsutsugamushi), the causative agent of scrub typhus — a life-threatening mite-borne rickettsial disease transmitted by the bite of infected larval trombiculid mites (chiggers). It is endemic across the Asia-Pacific region (tsutsugamushi triangle), including India, Southeast Asia, China, Japan, and Australia. The hallmark clinical finding is an eschar (painless, black, necrotic skin lesion at the mite bite site), found in 40–90% of cases. Clinical features include acute-onset fever, headache, myalgia, lymphadenopathy, rash (maculopapular, appearing on Days 5–8), hepatosplenomegaly, and in severe cases multiorgan failure, acute respiratory distress syndrome (ARDS), meningitis, and myocarditis. IgM antibody detection (by IFA, ELISA, or rapid immunochromatographic assay) is the primary diagnostic method; IgM titres ≥1:320 by IFA are considered positive. The traditional Weil-Felix test using OX-K antigen has poor sensitivity and specificity and is being replaced by antigen-specific methods. Doxycycline is the drug of choice (100 mg twice daily for 7–15 days); delayed treatment is associated with high mortality. Azithromycin is an alternative in pregnancy.
