Test Description

Dengue virus (DENV) has four serotypes (DENV 1–4), transmitted by Aedes aegypti and Aedes albopictus mosquitoes, causing an estimated 390 million infections annually in tropical and subtropical regions. Laboratory diagnosis of dengue depends on the timing of sample collection relative to illness onset. Dengue NS1 antigen (non-structural protein 1) is secreted into the bloodstream during active viral replication and is detectable from day 1 through approximately day 9 of illness, making it the preferred marker during the acute febrile phase. NS1 sensitivity: 70–90% in early primary infection; lower in secondary infections. Dengue IgM antibodies appear around day 4–5 of illness, peak at 2 weeks, and persist for 2–3 months. IgM detection indicates recent or current primary infection. Dengue IgG antibodies develop later (after day 7–10) and persist for life; their presence from early illness suggests secondary dengue infection (antibody titer boosted from previous DENV exposure). WHO recommended rapid diagnostic algorithm: Day 1–5 — NS1 antigen ± RT-PCR; After day 5 — IgM/IgG serology. Combined NS1 + IgM/IgG testing maximizes sensitivity across all illness stages.
