Creatinine clearance (CrCl) is a measure of the renal filtration capacity, used to estimate glomerular filtration rate (GFR). It is calculated from simultaneous measurement of serum creatinine and 24-hour urine creatinine and volume: CrCl (mL/min) = (Urine creatinine × Urine volume [mL/24h]) / (Serum creatinine × 1440). Creatinine is an endogenous metabolite of phosphocreatine from muscle metabolism, filtered freely at the glomerulus and partially secreted by renal tubules (approximately 10–15% of excreted creatinine is secreted, not filtered) — this causes CrCl to slightly overestimate true GFR. Normal CrCl: males 97–137 mL/min/1.73m²; females 88–128 mL/min/1.73m². The Cockcroft-Gault formula estimates CrCl without urine collection: CrCl = [(140-age) × weight(kg)] / (serum creatinine × 72) × 0.85 for females. CrCl is used clinically for: drug dosing adjustment in renal impairment (aminoglycosides, vancomycin, direct oral anticoagulants, carboplatin dosing using Calvert formula); CKD severity classification (eGFR staging); monitoring renal function in patients on nephrotoxic drugs. CrCl is decreasing in clinical importance as estimated GFR (eGFR) using the CKD-EPI equation has been validated and standardized.
