Fasting Blood Glucose (FBG) measures the concentration of glucose in blood after a minimum fasting period of 8 hours. Glucose is the primary energy substrate for cells and its homeostasis is tightly regulated by insulin (promoting cellular uptake and glycogen synthesis) and counter-regulatory hormones (glucagon, cortisol, epinephrine, growth hormone). Normal fasting glucose: 70–99 mg/dL (3.9–5.5 mmol/L). Prediabetes: 100–125 mg/dL (impaired fasting glucose). Diabetes mellitus: ≥126 mg/dL on two separate occasions, or once with symptoms. FBG is used for diabetes screening, diagnosis, and monitoring. In hospitalized patients, hyperglycemia (even without known diabetes) is associated with increased morbidity and mortality. Hypoglycemia (<70 mg/dL) can be symptomatic (<54 mg/dL) or clinically significant and may indicate insulinoma, adrenal insufficiency, liver failure, or iatrogenic causes (insulin or sulfonylurea therapy). FBG measurement uses enzymatic methods (glucose oxidase or hexokinase) on automated analyzers. Point-of-care glucometers are used for bedside monitoring but are less accurate than laboratory methods.
