Test Description
Anti-Müllerian Hormone (AMH) is a dimeric glycoprotein produced exclusively by granulosa cells of preantral and small antral follicles in the ovary, and its serum level is the most reliable and clinically validated marker of ovarian reserve — the quantitative reproductive potential of the ovary. Unlike FSH and oestradiol, AMH shows minimal intra-cycle and inter-cycle variation and can be measured on any day of the menstrual cycle. AMH levels decline progressively with age (peak ~25 years, significantly declined by age 37, undetectable at menopause) and correlate directly with antral follicle count (AFC) on transvaginal ultrasound. Low AMH (<1.0 ng/mL) indicates diminished ovarian reserve (DOR) and predicts a poor response to ovarian stimulation in IVF, increasing the risk of cycle cancellation and reduced oocyte yield. Very low AMH (<0.5 ng/mL) may indicate premature ovarian insufficiency (POI) in women under 40. Elevated AMH (>3.5 ng/mL) is associated with polycystic ovary syndrome (PCOS), reflecting the markedly increased number of small antral follicles. AMH is also used in granulosa cell tumour monitoring (AMH as a tumour marker in granulosa cell carcinoma). In males, AMH reflects Sertoli cell function; low AMH in prepubertal males may indicate anorchism or testicular dysgenesis.
