Test Description
Alkaline Phosphatase (ALP) is an enzyme found predominantly in the liver (bile canalicular membrane), bone (osteoblasts), intestine, placenta, and kidneys. Serum ALP is a critical marker in the evaluation of hepatobiliary and metabolic bone diseases. Normal range: 44–147 U/L in adults (significantly higher in children and adolescents due to bone growth). Elevated ALP with elevated GGT (gamma-glutamyl transferase) indicates hepatobiliary disease, particularly cholestasis (intrahepatic or extrahepatic). Conditions causing elevated hepatic ALP include primary biliary cholangitis (PBC), primary sclerosing cholangitis (PSC), cholestatic drug reactions, hepatic infiltrations (sarcoidosis, malignancy), and biliary obstruction. Elevated ALP with normal GGT suggests bony origin: Paget's disease of bone, hyperparathyroidism, osteomalacia, bone metastases, or healing fractures. ALP isoenzymes (hepatic, bone, intestinal, placental) can be measured to determine the source when ALP is elevated without clinical context. Physiologically elevated ALP is seen in the third trimester of pregnancy (placental ALP), puberty, and post-prandially (intestinal ALP in blood group B and O secretors). Low ALP (hypophosphatasaemia) is rare and is seen in hypothyroidism, pernicious anaemia, and the genetic condition hypophosphatasia.
