ACTH (Adrenocorticotropic Hormone) Test Overview & Clinical Guidelines

Test Description

Adrenocorticotropic Hormone (ACTH), also called corticotropin, is a 39-amino acid polypeptide produced and secreted by corticotroph cells of the anterior pituitary gland in response to corticotropin-releasing hormone (CRH) from the hypothalamus. ACTH stimulates the adrenal cortex to synthesize and secrete cortisol, as well as adrenal androgens and, to a minor extent, aldosterone. ACTH itself is regulated by negative feedback from cortisol. ACTH measurement is essential for distinguishing ACTH-dependent from ACTH-independent causes of adrenal dysfunction. In primary adrenal insufficiency (Addison disease): cortisol is low and ACTH is markedly elevated (>200 pg/mL) due to loss of negative feedback. In secondary adrenal insufficiency (pituitary or hypothalamic disease): both ACTH and cortisol are low. In Cushing syndrome: elevated ACTH indicates a pituitary or ectopic source; suppressed ACTH indicates an adrenal tumor (which autonomously secretes cortisol). Ectopic ACTH sources (small cell lung cancer, carcinoid tumors) produce very high ACTH levels (>200 pg/mL) with very high cortisol. Normal morning plasma ACTH: 10–60 pg/mL (2.2–13.2 pmol/L).
