Test Description
The Absolute Eosinophil Count (AEC) measures the total number of eosinophils per microliter of blood. Eosinophils are granulocytes derived from bone marrow progenitors and are characterized by their bilobed nucleus and cytoplasmic granules containing major basic protein (MBP), eosinophil cationic protein (ECP), eosinophil peroxidase, and eosinophil-derived neurotoxin. Normal AEC is 100–500 cells/µL. Mild eosinophilia: 500–1,500/µL; moderate: 1,500–5,000/µL; severe (hypereosinophilia): >5,000/µL. Eosinophilia is commonly associated with allergic conditions (asthma, allergic rhinitis), parasitic infections (especially tissue-invasive helminths such as Ascaris, toxocara, filaria), drug hypersensitivity, and skin conditions (eczema, pemphigus). Severe and persistent eosinophilia may indicate hypereosinophilic syndrome (HES), which can cause end-organ damage including cardiac fibrosis, neuropathy, and pulmonary infiltrates. Eosinopenia (AEC <100/µL) is seen in acute infections and corticosteroid therapy. AEC is more accurate than the differential percentage and is the preferred measure for clinical decision-making.
